REFUND POLICY:
NO CASH REFUNDS. (It means: no refunds on credit cards or checks. If by any personal reason a customer decides to withdraw from our services, a credit will be issued to the family in question with NO expiration date).MAKE UP CLASS POLICY:
A make up class will be scheduled for a participant only in case of illness or injury with written verification from a physician. A make up will be scheduled for an entire class if UAC cancels the class due to snow etc.BIRTHDAY PARTY POLICY:
Birthday party reservations require a $50 deposit. The balance of the party can be paid the day of the party. Please be sure of the date and time, no last minute changes or cancellations allowed. If by any personal reason a customer decides to withdraw from having a Birthday party two weeks before the date, a CREDIT IN FULL will be issued to the family in question, with no expiration date. *Sorry, no confetti or pinattas are allowed at our parties.FIELD RENTAL POLICY:
All team managers/person-in-charge must complete a form containing information such as: phone number (home and/or cell/work), address, etc. Field Rentals are limited to recreational or team training. Field Rentals cannot be outsourced to conduct private business. All players must sign a Release of liability form before going on the field. Payment for rental is requested by the time of booking. When paying by cash, always request a receipt (it should contain at least the date and time of rental). All checks should be payable to United Athletic Centers.For long term rental:
A deposit is required when booking the reservations. The deposit will be lost if you don’t show up for your rental or two week notice is not given for final rental. Any deliquency in payment will result in losing the time requested.League Play
All team managers/person-in-charge must complete a form containing information such as phone number(home and/or cell/work), address, etc. Team managers must collect atleast half the cost of the league from his/her team to pay United Athletic Centers before league play begins. The balance of league must be paid in full by mid session of the league.GENERAL RULES OF THE FACILITY
Individuals utilizing this facility do so at their own risk. The property owner(s), owner(s), and staff of United Athletic Centers (UAC) assume no liability for any injuries or accidents that may occur. Please reference Waiver and Release Forms and signage posted within the facility.
Conduct within the facility should be in the spirit of good sportsmanship as is on the field. Please NO ALCOHOL OR DRUG USE prior to playing. Should you find or lose any items, please report this immediately to the UAC staff. NO ALCOHOL, SMOKING, GLASS CONTAINERS, or GUM allowed in the facility. NO FOOD, DRINKS, SPITTING, OR ABUSIVE LANGUAGE ALLOWED ON THE FIELD. We reserve the right to refuse play or service to anyone.
